I am not sure I need home care. How do I set up an evaluation?

Call our office. We will be happy to arrange a meeting to discuss your medical and personal care needs and answer questions you may have.

What kind of services do you provide?

We provide skilled nursing, physical therapy, occupational therapy, speech therapy, medical social worker and home health aide services to patients in their home. Supportive services such as homemakers and companions, Lifeline installations, Adult Day Care and our Patient Home Monitoring Program are examples of non-medical services offered. A nurse is on-call 24 hours a day to answer questions or address non-emergency health situations that might occur.

How do I start services?

It’s easy! Referrals can be made by attending physicians, family members, hospitals, skilled nursing facilities, etc. We will need to obtain your physician’s approval for all medical services provided. You will be contacted to arrange a convenient time to have the nurse perform the initial assessment. At the time of the initial meeting, you will be informed of your rights and responsibilities and be given the opportunity to ask any questions you might have.

Tell me about your staff.

Personnel are screened to ensure they are qualified. All personnel caring for patients are subjected to criminal history and drivers license checks, reference verification and professional licensure and/or certification verification. Home Health Aides must complete a competency evaluation conducted by a Registered Nurse. All staff receives orientation and ongoing in-service education.

How are services scheduled?

We realize that it can be stressful having "strangers" come into your home. We try to minimize the number of people that come to your home by assigning each patient to a particular staff member. We ask our staff to pre-arrange their visits so that you are aware of the approximate times of their visits. Occasionally, a different staff member may need to visit you, but we try to keep it minimal.

What if I am receiving services from another home care agency?

We will evaluate your needs and contact the other provider to ensure appropriate care coordination and avoid prohibited duplication of services.

Who do I contact when I have questions regarding services?

Our patient's care is managed by his/her physician and provided by our professional home care team. There is a Registered Nurse on-call twenty four (24) hours a day, seven (7) days a week. Clients admitted for services are provided information regarding a contact person and how to reach them during regular office hours for questions or concerns.

How long has your company been in business?

Our agencies have been providing services in the community for as long as 95 years. Our staff visits patients of all ages. Most of our clients have medical need; others are in need of support services such as homemakers.

Is your Agency licensed and are you bonded and insured?

We are state licensed and Medicare/Medicaid certified. We are audited by The State Department of Health to ensure compliance with the regulations for home care agencies. We are insured as required by law.

What is covered by insurance?

Prior to providing care, we will review your insurance plan and confirm with you your level of coverage. Third-party payors may include Medicare, Medicaid, Veterans Administration, Private Insurance and grant programs. Benefits vary from plan to plan. Private Pay rates have been established for services not covered by a third party payor. We can provide you with specific information once we determine which payor will be billed for services.
