Established in 1995 by two registered nurses. For eleven years the agency has played an active role in the communities it serves by providing various health screenings. Allcare boasts that many of its employees have been with the agency since 1995, when the agency first began serving patients. The patients take some comfort in knowing that, often, the same nurse that cared for them five years ago is able to return to care for them or a loved one, when requested. Allcare is a small agency with a big heart!

Clinical Management Services, Inc.

Indiana

The agency was established in 1990. The founders saw that the health care needs of this rural community were great. A Home Health Agency operating under a community based service philosophy could play an important role in an effort to improve the health outcomes of residents with limited access to care. CMS is proud to offer health screenings to the public. Anodyne therapy is now available through the agency to those suffering from diabetic neuropathy, wounds which do not heal and intractable pain.

The agency was established in 1999 in an effort to continue services the health department based agency had been providing the community for 25 years.

New service programs are developed as need and opportunity is identified. We are one of the few agencies offering Anodyne therapy in all of West Central Michigan. Health Assessment and Blood Pressure Clinics are routinely offered to promote public health.

Established in 1988, we quickly earned a reputation as the agency “with heart” known for “keeping you in your home”. We achieved State licensure in 1991 as well as Medicare and Medicaid certification. The agency is unique in that our service area spans two states. We serve the Northwest corner of Indiana and the Southwest corner of Michigan.

Over half our employees have been with us for more than ten (10) years, four (4) since 1988. Our personnel are a measure of our commitment to achieving a level of care previously unattainable in our service area.

You will find us out in the community providing Blood Pressure screenings, Flu and Pneumonia vaccination clinics and cholesterol screenings in an effort to promote public health.

Community Home Health Care Services, LLC

Florida

Established in 2002, we are proud to have an Advanced Practice Nurse Practitioner at the helm. Florida presents a challenge to health care providers. The older population is far greater than in other states. One would believe access to health care would not be difficult because the need is great.

On the contrary, we have found that there is a significant population of underserved patients whose quality of life and medical condition would improve dramatically with timely intervention.

We celebrate the opportunity to provide home health services in our community.

Daybreak & Visiting Nurse Care, LLC

Indiana

Established in 1910, the first agency mission was serving poor mothers and children. Visiting Nurses soon found all adults needed guidance in proper hygiene and nutrition as well as when medical intervention was necessary. The focus shifted from mothers and children to the entire family. It was a perfect opportunity to improve the health of the nation’s poor.

Older adults were particularly at risk because they could no longer contribute to the work force. The efforts of health care practitioners tended to center around younger individuals and of course, those who could pay.

Lyndon Johnson’s Great Society opened the door for older adult access to health care, regardless of ability to pay. The Medicare Home Health Benefit was established in the original Social Security legislation. Our agency became a Certified Medicare provider in 1976 and has served Medicare beneficiaries ever since.

A commitment to meeting needs in the community has been the driving force as we develop new programs of service. Adult Day Care, Lifeline and supportive services programs have been established with great success.
